PATIENT'S RESPONSIBILITIES

  1. 1. Providing the Admitting/Emergency Room sections with all information necessary for documentation and to correctly determine eligibility or to bill insurance companies.
  2. 2. Providing the health care personnel in attendance with accurate, complete information regarding their medical and family history, medication regimen, medication allergies, surgical history, or any information relevant to their medical or emotional condition.
  3. Complying with the rules and regulations of CDUH and accepting full responsibilities for the payment of charges incurred.
  4. Respecting the property of CDUH.
  5. Making it known whether or not they completely understand their diagnosis, treatment plan, follow up instructions, medication regimen, and what is expected of them.
  6. Following the physician’s recommendation for further care, diet, tests or medications, etc.
  7. Reporting any changes in their condition to the responsible practitioner.
  8. Accepting the consequences of refusing treatment or not following instructions.
  9. Patient should feel free to ask their health care provider for additional visual or auditory privacy at any time.
  10. Notifying health care personnel of any disability or barrier, which interferes with communications, transportation, etc.
  11. Being considerate of the needs of others especially assisting in control of noise and limiting visitors.
  12. Recognizing the impact of life style on their personal health.
  13. Keeping their follow up clinic and diagnostic study appointments or notifying the appropriate personnel when they are unable to comply.
